Hi, I was in the Navy for a short time last year and I would like to obtain my medical record. I just want to know what immunizations I received in boot camp so I can get them added to my shot record. Where can I obtain this information?

Hi, I worked in a Medical Records department in a Navy clinic for a while...go to your nearest Navy/MC base clinic, and they should have forms available for you to request your medical records to be sent to that facility, or a copy mailed to you. It's a long drawn out process though, I hate to say that a lot of time, people came in 6 months later and wanted to know where their records were, and they weren't ever found. Med Recs tries to run a tight ship but after they send out the separated AD records, it's out of their hands. If it wasn't *that* long ago, maybe you will be lucky and the clinic you were treated at will still have them there. Sometimes there is a back log that keeps them from being sent out immediately upon separation of the individual. Hope this information helps, Med Recs should be able to handle everything for you.
